**This is the preferred option as the school will take a percentage off donations made online through their website.**
Step 1:
Write a check made payable to "ASI" and include the player that you are donating on behalf of in the bottom left. If it is just a general donation, please leave that blank.
Step 2:
Mail the check to the address below.

Scroll down and write the dollar amount in the club box (second from the bottom).
Step 3:
Write "Cal Poly Men's Volleyball Club" in the box below, the name of the player that you are donating on behalf of in the "Special Requests" box, and fill out the following information.
